How to Submit Your Insurance Successfully

Always call your insurance provider to confirm benefits. Patients are responsible for any deductibles, copayments, cost shares, coinsurance, or balance. If for any reason your insurance company deems your lab work as a non-covered or medically unnecessary service, you will be responsible for the full amount of the charge.

Required Checklist Before Shipping Specimen(s)

  • Test Requisition Form (all fields in red)

           -  ICD-10 Codes
           -  Physician Signature
           -  Patient/Guarantor Signature
           -  Signed Acknowledgement of Privacy Practices

  • Photocopy of Insurance Card (both sides)

  • 20% prepayment for all out-of-network plans (see)
