Payment Options
You have several options for payment when testing with The Great Plains Laboratory:
Check or Credit Card
Take advantage of our prompt pay discount by paying in full at the time of testing. We accept checks and all major credit cards including: Visa, Master Card, Discover, American Express, JCB, and Diners Club. We will send a statement of services for you to file with your insurance company.
Using Insurance (U.S. only)
If you choose to have The Great Plains Laboratory file a claim with your insurance company, photocopy your insurance card (both sides) and attach to it to your test requisition.
Enclose a 20% prepayment of the total (list) cost and we will make a claim to your insurance company for you. There are a few tests we cannot bill insurance for; make sure to check our price list before sending your payment. This prepayment is not necessary for Blue Cross Blue Shield PPO members or for Tricare and Medicare patients
We file insurance claims as a courtesy to our clients and we will help you receive maximum benefits. However, please remember your insurance is a contract between you and your insurance company and we are not a party to this contract. We cannot become involved in disputes between you and your insurance company regarding deductibles, co-payments, covered charges, secondary insurance, or any other issue beyond supplying factual information as necessary. We also do not bill insurance after the testing fee has been paid by cash or credit card, but we will mail you a reimbursement form for you to send your insurance company if you request it.
If we have not received payment from your insurance company within 60 days, we will bill you for the test.
Payment is due from you at that time. If we receive a payment from your insurance company after you have already paid for a test, we will return the insurance payment directly to you.
Insurance Plans not covered
We cannot bill any state Medicaid plans or HMO's. If you have these insurance carriers you must pay in full by check or credit card. A receipt will be sent to you.
Blue Cross and Blue Shield Claims
You are a member of Blue Cross Blue Shield (BCBS) Blue Card PPO anywhere in the United States as long as your membership card has a suitcase logo on it.
OR
You are a member of BCBS Federal Employee Program. Your card will have a map of the United States on it.
Claim processing will apply as follows:
• You must photocopy your membership cards (both sides) and submit with your requisition.
• We will not process your lab results without appropriate ICD-9 diagnosis code and signature provided by your physician.
• We will not accept any payment from you in advance of service except for the IgG food allergy test - payment is required for the IgG for BCBS patients”.
• Your doctor does not need to be a Blue Cross provider of any type.
• Once we receive payment information from BCBS you will be responsible for any deductible and/or coinsurance.
• If for any reason BCBS deems your lab work as a non-covered or medically unnecessary service, you will be responsible for the full amount of the charge.
• We will not process your lab results without proper signature of acknowledgment on this form below and on the test requisition.
Tricare South
All patients who have Tricare South insurance are required to fill out a non-covered services waiver form. The form is found here.
Requirements
The approval of someone legally acceptable to order tests in your state is necessary for us to do testing. If the doctor is not available to sign the requisition form, have him/her fax the order to us on a script pad paper. The authorized person may vary from state to state and may include chiropractors, D.O.'s, naturopaths, nurses, dentists, etc.
For Medicare and insurance claims, the physician must record the appropriate ICD-9 diagnosis code on the requisition or on the script pad order. The physician must also record his Medicare provider number (called a UPIN number) for Medicare claims. We accept Medicare payment as complete reimbursement for our testing.
If you do want us to file an insurance claim for you, please remember to photocopy both sides of your insurance card and send it to us.
