Please contact a customer service representative if you have any questions. You have two options for payment when testing with The Great Plains Laboratory:
Using InsuranceIf you choose to have The Great Plains Laboratory make a claim to your insurance company, photocopy your insurance card (both sides) and attach to it to your test requisition. Make sure the address on your insurance card is the correct address for claims. Sometimes claims go to a different address, and verifying the address first will help the entire claims process run much more quickly and efficiently. We file insurance claims as a courtesy to our clients and we will help you receive maximum benefits. However, please remember your insurance is a contract between you and your insurance company and we are not a party to this contract. We cannot become involved in disputes between you and your insurance company regarding deductibles, co-payments, covered charges, secondary insurance, or any other issue beyond supplying factual information as necessary. We also do not bill insurance after the testing fee has been paid by cash or credit card, but we will mail you a reimbursement form for you to send your insurance company if you request it. If we have not received payment from your insurance
company within 60 days, we will bill you for the test. Insurance Plans not covered We cannot bill any state Medicaid plans, GHI or HMO's. If you have these insurance carriers you must pay in full by check or credit card. An insurance form for reimbursement will be sent to you. Blue Cross and Blue Shield ClaimsYou are a member of Blue Cross Blue Shield of Kansas City (BCBSKC) PPO Greater Kansas City and Northwest Missouri. Counties in Missouri include Andrew, Atchison, Bates, Benton, Buchanan, Caldwell, Carroll, Cass, Clay, Clinton, Daviess, DeKalb, Gentry, Grundy, Harrison, Henry, Holt, Jackson, Johnson, Lafayette, Livingston, Mercer, Nodaway, Pettis, Platte, Ray, Saline, St. Clair, Vernon, and Worth. Also included are Johnson and Wyandotte counties in Kansas. ORYou are a member of Blue Cross Blue Shield (BCBS) Blue Card PPOAnywhere in the United States as long as your membership card has a suitcase logo on it. This may include many counties in Kansas or Missouri not listed as part of Greater Kansas City above. Claim processing will apply as follows: You must photocopy your membership cards (both sides) and submit with your requisition. We will not process your lab results without appropriate ICD-9 diagnosis code and signature provided by your physician. We will not accept any payment from you in advance of service. Your doctor does not need to be a Blue Cross provider of any type. Once we receive payment information from BCBS you will be responsible for any deductible and/or coinsurance. If for any reason BCBS deems your lab work as a non-covered or medically unnecessary service, you will be responsible for the full amount of the charge. We will not process your lab results without proper signature of acknowledgment on this form below and on the test requisition. This applies to all lab service rendered on or after July 1, 2004. RequirementsThe approval of someone legally acceptable to order tests in your state is necessary for us to do testing. If the doctor is not available to sign the requisition form, have him/her fax the order to us on a script pad paper. The authorized person may vary from state to state and may include chiropractors, D.O.'s, naturopaths, nurses, dentists, etc. For Medicare and insurance claims, the physician must record the appropriate ICD-9 diagnosis code on the requisition or on the script pad order. The physician must also record his Medicare provider number (called a UPIN number) for Medicare claims. We accept Medicare payment as complete reimbursement for our testing. If you do want us to file an insurance claim for you, please remember to photocopy both sides of your insurance card and send it to us.
