The Great Plains Laboratory, Inc. will file insurance claims as a courtesy to our United States clients and will help the patient receive maximum benefits. However, please keep in mind that the insurance policy is a contract between the patient and the insurance company. The laboratory cannot become involved in disputes between the patient and the insurance company regarding deductibles, co-payments, covered charges, secondary insurance, or any other issue beyond supplying factual information as necessary. The laboratory will not bill insurance after the test has been paid by cash or credit card. In these cases, the laboratory will mail a reimbursement form to the patient which can be submitted to the insurance company for reimbursement.
If the laboratory has not received payment from the insurance company within 60 days, the patient will be billed for the test.
Payment is due from you at that time. If payment is received from the insurance company after the patient has already paid for a test, the laboratory will return the insurance payment directly to the patient.
Exceptions
BCBS: The Great Plains Laboratory is no longer "in-network" with BCBS of Florida, BCBS of Montana, and Premera Blue Cross of Washington and Alaska. The Great Plains Laboratory will require upfront payment in full for all members of the BCBS plans listed above.
IgG Food Allergy Test w/ Candida: Anthem BCBS of California and BCBS of North Carolina subscribers will be required to pay for the test in full at the time of service. Patients may find it financially beneficial to order the Organic Acids Test + IgG Food Allergy Test w/ Candida Combo.
Some BCBS plans do not cover the IgG Food Allergy Test w/ Candida. Please call your BCBS plan to confirm coverage for this test. The CPT code to check is 86001. If the test is not covered, the patient will be responsible for the balance in full.
Medicare: Payment in full (patient cash price) is required for the Comprehensive Fatty Acids Test, Gluten/Casein Peptides Test, IgE Allergy Advanced Combined Test, IgE Allergy Food Allergy Tests, IgE Inhalant Allergy Tests, IgG Food Allergy Test w/ Candida, Metals Fecal Test, Metals Hair Test, Metals Red Blood Cell Test, Metals Urine Test, Metals Whole Blood Test, and the Vitamin D Test by Medicare patients. These tests are not eligible to be filed with Medicare.
