You have two options for payment when testing with The Great Plains
Laboratory:
Take advantage of our cash discount by paying in full at the
time of testing. We will send a statement of services for you
to file with your insurance company.
Enclose a 20% prepayment of the total (insurance) cost and we
will make a claim to your insurance company for you. There are
a few tests we cannot bill insurance for; make sure to check the our
price list before sending your payment.
If you choose to have The Great Plains Laboratory
make a claim to your insurance company, photocopy your insurance card
(both sides) and attach to it to your test requisition.
Make sure the address on your insurance card is the correct address for
claims. Sometimes claims go to a different address, and verifying the
address first will help the entire claims process run much more quickly
and efficiently.
We file insurance claims as a courtesy to our clients and we will help
you receive maximum benefits. However, please remember your insurance
is a contract between you and your insurance company and we are not a
party to this contract. We cannot become involved in disputes between
you and your insurance company regarding deductibles, co-payments, covered
charges, secondary insurance, or any other issue beyond supplying factual
information as necessary. We also do not bill insurance after the testing
fee has been paid by cash or credit card, but we will mail you a reimbursement
form for you to send your insurance company if you request it.
If we have not received payment from your insurance
company within 60 days, we will bill you for the test.
Payment is due from you at that time. If we receive a payment from your
insurance company after you have already paid for a test, we will return
the insurance payment directly to you.
We do not bill Blue Cross and Blue Shield or their
affiliates in any state or Medicaid. If your insurance is Blue
Cross and Blue Shield or Medicaid, you must pay by cash or credit
card. We will send you a statement of service that you may use for reimbursement.
Most Blue Cross and Blue Shield Plans have reimbursed their insured customers
for our testing, but we cannot guarantee reimbursement.
All Blue Cross and Blue Shield Claims (from every state)
must include our Provider Number, 23891011, and must be filed at the same
address:
Card Blue ITS
Blue Cross & Blue Shield of Kansas City
PO Box 419016
Kansas City, MO 64141-6016
The approval of someone legally acceptable to
order tests in your state is necessary for us to do testing. If
the doctor is not available to sign the requisition form, have him/her
fax the order to us on a script pad paper.
For Medicare and insurance claims, the physician
must record the appropriate ICD-9 diagnosis code on the requisition or
on the script pad order. The physician must also record his Medicare
provider number (called a UPIN number) for Medicare claims. We accept
Medicare payment as complete reimbursement for our testing.
If you do want us to file an insurance claim for you, please remember
to photocopy both sides of your insurance card and send
it to us.
